SCVH Patient Consent to Receiving Telephone Calls and Texts

Patients consent to receiving calls (including autodialed calls and artificial or prerecorded messages) and text messages by providing a landline or wireless phone number to Santa Clara Valley Healthcare (SCVH). Messages may be sent from SCVH, as well as its physicians, agents, independent contractors (including service agencies), and affiliated County of Santa Clara Departments including the Department of Tax and Collections. Messages may be sent about medical services, related financial obligations, and other health-related benefits and/or services.
